A 2-minute lifestyle interview covers the nine factors with the strongest evidence-based
link to biological aging: exercise frequency, sleep quality, diet, body composition,
smoking status, alcohol consumption, stress levels, chronic conditions, and family longevity.
Your answers are processed through the Immortality Index® algorithm, adjusted for your
age, geographic region, and urban environment, to produce an estimated biological age
and a score from 0 to 400.

Level 2 incorporates biometric data from your wearable device. Enter your
heart rate variability, resting heart rate, VO2 max, step counts, sleep patterns, and
recovery metrics manually from your Apple Watch, Garmin, WHOOP, Oura, or any wearable.
These objective measurements replace self-reported estimates with real data, and
the maximum scoring range increases to 600.

Level 3 — Blood Biomarkers

Level 3 is the complete Immortality Index®. You enter six standard blood biomarkers
from any routine blood panel: HbA1c (glycated hemoglobin), fasting glucose, triglycerides,
HDL cholesterol, total cholesterol, and hs-CRP (high-sensitivity C-reactive protein).
These markers cover metabolic health, cardiovascular risk, and systemic inflammation — the
three pillars of biological aging. Combined with your lifestyle data and wearable metrics,
the full algorithm produces a score from 0 to 800 with near-clinical accuracy.

A Level 1 user with a score of 520 ranks alongside a Level 3 user with a score of 870.
